Job summary

Southwest Behavioral & Health Services is seeking a Housing Specialist for our HOPWA program in Phoenix, AZ. This role includes facilitating eligibility screenings and housing placements for individuals and families affected by HIV/AIDS. You will develop Individualized Housing Plans and ensure compliance with housing standards.

The ideal candidate will have a high school diploma or GED along with relevant work experience, a valid Arizona driver's license, and fingerprint clearance from the Arizona Department of Public Safety. Join us to make a meaningful impact in the community.

Qualifications

• At least 21 years of age.
• Two years of behavioral health/integrated healthcare relevant work experience.
• Valid Arizona driver’s license.
• Fingerprint clearance from the Arizona Department of Public Safety.

Responsibilities

• Facilitate eligibility screening for individuals/families infected and affected with HIV/AIDS.
• Develop Individualized Housing Plans with residents.
• Maintain hygiene and safety standards of all units.
• Consult with landlords regarding unit responsibilities.
