Job summary

Qualifications

• Two (2) years of case management or direct outreach experience.
• Graduation with a two-year degree in Addiction Counseling, Psychology, Social Work, or related field.
• Bilingual preferred; strong communication and confidentiality.

Responsibilities

• Conduct outreach and navigation across Fresno-Madera County.
• Intake and assessment to determine eligibility and services.
• Manage a caseload with service plans and data tracking.
• Build relationships with property managers and perform warm handoffs.
• Provide linguistically and culturally appropriate services.
