UnitedHealth Group's Optum HouseCalls program is hiring Advanced Practice Clinicians (NP/PA) to conduct in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers.

Flexible scheduling between 20-40 hours per week with no on-call, weekends, or holidays. Competitive compensation, sign-on bonus options, and comprehensive benefits are offered to eligible candidates.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Gerontology Primary Care
• Current, unrestricted state licensure to practice as NP/PA
• Driver's license and reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments

Responsabilités

• Conduct in-home assessments including vital signs, history, exam
• Document diagnoses for care management and treatment planning
• Communicate findings to primary care providers to address gaps in care
• Identify urgent situations and intervene appropriately
• Educate members on disease processes and medications
• Address social determinants of health and provide referrals as needed
