Job summary

Optum HouseCalls is hiring Advanced Practice Clinicians (NPs or PAs) to perform in-home health assessments for Medicare Advantage and other members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes and reduce hospitalizations.

Must hold active NP/PA certification and state licensure, with unrestricted credentials and a valid driver's license.

Qualifications

• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date)
• Active and unrestricted driver's license
• Ability to navigate varied home environments and conduct assessments

Responsibilities

• Conduct comprehensive in-home assessments including history, medications, vitals and exam
• Document diagnoses for care management and treatment planning
• Communicate findings to PCPs to close care gaps
• Identify urgent situations and intervene as needed
• Educate patients on disease processes, medications, and adherence
• Address social determinants of health and provide referrals
