Job summary

Optum Home & Community Care is hiring HouseCalls Advanced Practice Clinicians (NPs or PAs) to perform annual in-home health assessments for Medicare Advantage members. This field-based, non-prescribing role focuses on education, gap closure, and collaboration with PCPs.

Active certification and licensure are required, along with a valid driver’s license and ability to travel to patient homes. No on-call duties, flexible daytime hours, and a competitive compensation package are provided.

Qualifications

• Active and unrestricted licensure in the practicing state by start date.
• Active and unrestricted driver's license.
• Ability to obtain board approval if required and begin eligibility within timelines.
• New hires must pursue required approvals if allowed to practice independently.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to address gaps in care.
• Identify urgent or emergent situations and intervene as needed.
• Educate members about diseases, medications, and adherence.
• Address social determinants of health and refer to services as appropriate.
