Job description

Primary Vascular Care is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to perform on-site clinical assessments for residents of client facilities. This position reports to the Lead Nurse Practitioner.

The NP/PA will work in a mobile practice, primarily serving a long-term care setting.

Essential Functions:

1. Perform on-site screening assessments to determine appropriate patient treatment plans by completing comprehensive assessment forms.
2. Conduct independent history and physical exams for complex acute, critical, and chronically ill perioperative patients.
3. Order and review diagnostic and therapeutic tests based on patient needs.
4. Assist in providing care in accordance with facility, state, and federal regulations.
5. Monitor the effectiveness of interventions.
6. Maintain relationships with facility contacts.
7. Educate family members, clients, and prospective accounts about services offered by Primary Vascular Care.
8. Assist in developing and maintaining physician relations as assigned.
9. Complete reports timely and comprehensively.

Qualifications:

1. Bachelor's Degree required.
2. Degree from an accredited Nurse Practitioner or Physician Assistant Program required.
3. Current NP License required.
4. BLS certification through American Heart Association required.
5. ACLS certification preferred.
6. Current Infection Control Certification required.
7. Board Certified/Board Eligible required.
8. Physical exams, vaccines, and titers including PPD/TB, Hep B, MMR, Varicella required.
9. COVID and Flu vaccines preferred.
10. At least 1 year of NP experience preferred.
