Job summary

System Support seeks a Medical Social Worker for Miami Valley Hospital's Comprehensive OP Rehab Program. Provides direct social work intervention, transition planning and counseling to patients and families in inpatient/outpatient settings.

Responsibilities include assessing social, emotional, financial and family needs, coordinating with physicians and care teams, and linking to community resources. LSW required; LISW preferred; Master’s in Social Work required.

Qualifications

• Master's degree in Social Work required or strongly preferred.
• Licensed Social Worker (LSW) licensure required; LISW preferred.
• Strong interpersonal and written communication skills.

Responsibilities

• Provide direct social work intervention, transition planning and counseling to patients and families.
• Assess social, emotional, financial, cultural and family needs and communicate with care team.
• Assist with community resource linkage and transition planning for inpatient and outpatient populations.
