The Ohio State University Wexner Medical Center seeks a social worker to deliver psychosocial services to inpatients and outpatients and their families at University Hospitals East and affiliated clinics.

The role includes psychosocial assessments, counseling, crisis intervention, discharge planning, financial counseling, health education, and linking patients with community resources to support safe transitions between care environments.

Qualificações

• Master of Social Work from an accredited Graduate School of Social Work; State of Ohio Social Work licensure (LSW or LISW) or equivalent educational experience.
• Final candidates are subject to successful completion of a background check. A drug screen or physical may be required during the post offer process.

• Provide psychosocial assessments, counseling and crisis intervention to inpatients and outpatients and their families.
• Discharge planning, financial counseling and linkage with community resources.

Job DescriptionProvision of social work services to inpatients and outpatients and their families at University Hospitals East and clinics. Social work services include psyschosocial assessments, mental health counseling, crisis intervention, financial counseling, discharge planning, health education and the provision of material resources and linkage with community agencies. Social work services are provided to patients/families to meet their medically related social and emotional needs as they impact on their medical condition, treatment, recovery and safe transition from one care environment to another. Core competencies include : Psychosocial assessment, counseling and crisis intervention skills, Knowledge of the PASAAR process for extended care facility placement and the assessment and counseling processes involved, and skills assessing neglect/abuse, including substance abuse and domestic violence.Minimum Education RequiredMaster's Level DegreeRequired QualificationsMaster of Social Work from an accredited Graduate School of Social Work; State of Ohio Social Work licensure (LSW or LISW) or equivalent educational experience.
