Job summary

System Support seeks a Medical Social Worker for Miami Valley Hospital's Comprehensive OP Rehab Program. Provides direct social work intervention, transition planning and counseling to patients and families in inpatient/outpatient settings.

Responsibilities include assessing social, emotional, financial and family needs, coordinating with physicians and care teams, and linking to community resources. LSW required; LISW preferred; Master’s in Social Work required.

Qualifications

• Master's degree in Social Work required or strongly preferred.
• Licensed Social Worker (LSW) licensure required; LISW preferred.
• Strong interpersonal and written communication skills.

General Summary/Responsibilities:Status: Full-time: *40 Hours per week (80 hours per pay period), variesDepartment: COMPREHENSIVE OP REHAB PROGRAMFacility: Miami Valley HospitalThe medical social worker provides direct social work intervention, transition planning and counseling to patients and their families. The medical social worker assesses the patient’s social, emotional, financial, cultural and family situation and interprets this information to the physician, nursing staff and other care team members to facilitate appropriate transition planning. The medical social worker assists patients and families in making appropriate linkages to community resources and provided consultation to those seeking input and guidance in efforts to resolve patient concerns.
