Qualifikationen

• Experience with psychosocial assessment, short-term counseling, and crisis intervention.
• Knowledge of discharge planning and state/county protective services mandates.
• Ability to coordinate with hospital admins, medical teams, law enforcement, APS/DCS, schools, probation, and community agencies.

• Meet face-to-face with patients and families to provide psychosocial assessments and therapeutic interventions.
• Collaborate with health-care team to ensure safe, timely services and educate families about resources.
• Interpret complex needs and model caring practices for caregivers.

• Counsel patients and families on emotional, social, and financial impacts of illness.
• Mobilize family and community resources to meet needs.
• Provide mental health assessments on physician referral.
• Facilitate complex discharge planning and transitions to next level of care.

• Coordinate with state and county protective teams and guardianship/adoptions.
