Qualifications

• Master's degree in Social Work (CSWE accredited) required.
• Clinical licensure desirable; ACM-SW preferred.
• Experience in healthcare case management preferred.

Responsibilities

• Identify discharge needs and coordinate psychosocial planning.
• Assess data via interviews to determine patient needs.
• Develop and implement psychosocial treatment plans.
• Coordinate referrals to post-discharge services.
• Document in the EMR per departmental standards.
• Collaborate with the multi-disciplinary team to improve patient outcomes.
• Advocate for unmet patient needs and payer considerations.
• Participate in discharge planning to drive care progression.
