Job summary

IU Health Fishers Hospital seeks an experienced Social Worker - MSW LCSW Senior to provide advanced clinical social work services, focusing on complex discharge planning, crisis intervention, and mental health assessment.

This senior role also offers supervision to social workers pursuing LCSW licensure, serves as field instructor for students, and collaborates with hospital teams and community agencies to ensure safe, timely, and well-coordinated care.

Qualifications

• Master's Degree in Social Work required.
• Permanent LCSW licensure required.
• 5 years of experience in child welfare, mental health/addictions, health, school, or community/organizational settings.
• Crisis Prevention Intervention certification within 6 months of hire.
• Knowledge of psychosocial assessment and crisis intervention for patients and families with common mental health and substance use disorders.
• Ability to coordinate complex discharge planning to ensure safe discharge to the next level of care.
• Knowledge of state and county adult and child protective legal mandates.
• Strong collaboration with hospital administrators, medical teams, law enforcement, APS/DCS, schools, probation, and other community agencies as indicated.
• Computer literacy and proficiency documenting in an electronic medical record.
• On-call and holiday rotation may be required.

Responsibilities

• Meets face-to-face with patients and families.
• Acts as a preceptor to develop educational materials for team members, patients, and families.
• Provides clinical supervision for social workers pursuing LCSW licensure.
• Serves as a field instructor for social work students.
• Builds relationships with hospital administrators and medical teams to drive quality and process improvement.
• Participates in developing treatment plans to resolve psychosocial issues and consults on mental health diagnoses.
• Communicates with the health care team to facilitate the episode of care and ensure resources and education are provided.
• Interprets patient/family needs and models caring practices to caregivers.
• Assesses psychosocial risk factors and coping abilities of patients and families.
• Counsels patients and families on emotional, social, and financial consequences and mobilizes resources.
• Provides mental health assessments of patients upon physician referral.
• Provides interventions to facilitate complex discharge planning.
• Acts as liaison with child and adult protective teams.
• Maintains crisis intervention and de-escalation practices.
• Provides guardianship, foster care, adoption, and related mental health placements.
• Offers specialized mental health services including crisis intervention and addiction assessments.
• Leads clinical interventions in dangerous situations and crisis settings.
• Supports patients during acute crisis and uses de-escalation techniques to prevent violence.
• Uses interviewing principles with victims, perpetrators, and families.
• Delivers end-of-life care and related guardianship/adoption support.
