Job summary

HonorHealth is seeking a Case Management Social Worker to manage an assigned caseload and coordinate care across inpatient and outpatient settings. The role emphasizes smooth discharge planning, multidisciplinary collaboration, and advocacy for patients and families.

Required Masters in Social Work and licensed social work credentials, with preferred experience in case management and behavioral health. Day shifts, sign-on bonus up to $5,000, located at Osborn Medical Center in Scottsdale, AZ.

Qualifications

• Masters in Social Work required.
• 1 year as a Licensed Social Worker, and/or successful completion of health related field placement in Master's level Social Work Program required.
• 2 years in case management preferred.
• 3 years as a Licensed Social Worker preferred.
• Other Experience in management of behavioral health patients preferred.

Responsibilities

• Collaborates with patients/caregivers early in the inpatient and/or outpatient episode to coordinate discharge planning, including home visits and community resources.
• Facilitates communication of the plan of care between hospitalists, specialists and PCP, and notifies payer for authorization as needed.
• Maintains caseload and contributes to Case Management metrics to improve patient outcomes and cost effectiveness.
• Advocates for patients using applicable laws, regulations, benefits, and guidelines to ensure high quality, ethical care.
