Qualifications

• Must have MSW and Arizona license (LMSW/LCSW).
• Minimum 4 years of healthcare social work experience; value-based care preferred.
• Experience conducting home visits and addressing psychosocial barriers.

Responsibilities

• Partner with Discharge Planners and Care Managers to tailor transitions.
• Conduct home visits for psychosocial assessments and care planning.
• Provide discharge planning, referrals, and supportive counseling.
• Document encounters in the EMR timely and accurately.
• Collaborate with RN, NP, PCP, Behavioral Health, Pharmacy, and partners to support whole-person care.
• Support transitions from hospital/ED/SNF to home and prevent readmissions.
• Educate members and caregivers on condition management and community resources.
• Follow PPE standards and reporting requirements during visits.
