Job summary

Allina Health’s System CM Hospital Care Management team seeks a social worker to provide psychosocial support to patients and families facing chronic, acute, or terminal illness. The role involves counseling, education, care coordination, and connecting patients to community resources to improve access and outcomes.

The position is .6 FTE (48 hours per two-week pay period) on an 8-hour day shift with weekend rotation (1 in 6) within the MNA union. Licensure and state requirements apply.

Qualifications

• Bachelor's degree in Social Work.
• 0 to 2 years of experience in a health care setting.
• Master's degree in Social Work preferred.
• Licensed Social Worker required by MN Board of Social Work.

Responsibilities

• Delivers psychosocial support to patients and families.
• Provides counseling, education, and care coordination.
• Makes referrals for other services and supports patient access to care.
• Coordinates transition plans with care teams and community providers.
• Advocates for patients and families and reports safeguarding concerns as required.
• May participate in care system processes to prevent readmissions.
