Job summary

SJMH is seeking a part-time Social Worker to support patients and families facing severe, long-term illness. You will conduct interviews, prepare psychosocial assessments, develop treatment plans, provide counseling, and coordinate care across discharge planning and transitions of care within the interdisciplinary team.

Requirements include a Master’s in Social Work and Michigan licensure eligibility; experience is preferred but not required.

Qualifications

• Master's Degree in Social Work from an accredited program.
• Current licensure (MSW) in Michigan or eligibility.
• Experience preferred but not required.

Responsibilities

• Interviews patients and families to assess psychosocial needs.
• Develops treatment plans and provides counseling and crisis intervention.
• Coordinates care across discharge and transitions to post-acute networks.
• Collaborates with care teams to ensure quality and cost-effective care.
• Advocates for patient rights and connects to community resources.
