This role provides essential psychosocial support to patients and families navigating chronic, acute, or terminal illness. Through counseling, education, care coordination, and connection to community resources, the position helps reduce barriers to care, strengthen coping and resilience, and promote equitable, patientcentered health outcomes across the continuum of care.
Provides patients and families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family caregivers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.
Pay Range: $37.16 to $46.45 per hour The pay described reflects the base hourly pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
