Job summary

Allegheny Health Network seeks a social work professional to collaborate with an interdisciplinary care team, advocate for patients and families, and coordinate care across inpatient and post-discharge settings in alignment with social determinants of health.

This role emphasizes transition planning, patient education, crisis intervention, and participation in department and hospital committees. Master’s in Social Work and PA LSW/LCSW eligibility are required.

Qualifications

• Experience in a hospital or health care setting.
• Master's degree in Social Work; timeline to obtain licensure/eligibility.
• PA LSW and/or LCSW required; timeline if applicable.
• Act 34, Act 33, Act 73 background clearances may be required.
• HIPAA/privacy compliance expectations.

Responsibilities

• Contributes to initial and ongoing comprehensive assessments and coordinates care.
• Delivers appropriate discharge planning and connects to post-acute resources.
• Educates patients and families about post-discharge levels of care and access to support.
• Provides crisis intervention for abuse, mental health, and other concerns as needed.
• Supports professional development and participates in committees.
• Contributes to department and organizational goals.
• Other duties as assigned.
