Qualifications

• Provides psychosocial assessment and intervention planning.

• Advocates for patients and families and supports patient rights.

Responsibilities

• Delivers professional social work services, including psychosocial assessment and intervention planning.
• Maintains knowledge of government and private payer networks to assure appropriate transitions.
• Collaborates with interdisciplinary team to move the plan of care forward.
• Ensures complete clinical handoffs for at-risk patients and timely referrals.
