You will help patients and families navigate transitions, advocate for rights, and connect them with community resources.

The role emphasizes collaboration with an interdisciplinary team, documentation in EMR, and maintaining knowledge of payer networks.

Qualificações

• Bachelor's degree in Social Work.
• Licensed Social Worker in MN; WI license required if operating in Wisconsin or with Wisconsin patients remotely.
• 2+ years social work experience preferred.

Responsabilidades

• Delivers professional social work services including psychosocial assessment and intervention planning.
• Identifies patients needing social work assessment via high risk screening and referrals.
• Conducts bio-psycho-social assessments for individualized plans of care.
• Provides support and counseling to patients and families.
• Advises on financial or social needs and assists with referrals.
• Coordinates complex transition plans with patients, families, and healthcare teams.
• Uses EMR to monitor progress and communicate care plans.
