Job summary

AdventHealth in Orlando, FL is seeking a per diem Night Shift Social Work Case Manager to provide grief counseling, discharge planning and care coordination across inpatient, observation, and ED settings.

You will assess patient and family needs, develop post-acute plans, collaborate with post-acute services, and support families with emotional, social, and financial impacts of illness.

Qualifications

• Master’s in Social Work is required.
• Minimum 2+ social work experience.
• Minimum 2+ care management experience preferred.
• ACM/CCM licenses preferred.

Responsibilities

• Provides grief counseling, disease adjustment support, crisis intervention and goals of care planning.
• Assesses discharge planning needs in inpatient, observation or ED settings to avoid delays.
• Reviews medical records and integrates clinical, social and financial factors into care plans.
• Develops discharge plans and coordinates post-acute care services and facilities.
• Communicates with post-acute services to ensure continuity of care and complete records.
• Participates in multi-disciplinary rounds to identify resources for discharge and patient empowerment.
