Qualifications

• Master's degree in Social Work required.
• PA LSW and/or LCSW required.
• Background clearances: Act 34, Act 33, Act 73, FBI fingerprinting.

Responsibilities

• Contributes to initial and ongoing comprehensive assessments.
• Provides interventions and coordinates care with patients and families.
• Ensures safe transitions across inpatient, observation, and ED settings.
• Educates patients and families on level of care and post-discharge resources.
• Offers crisis interventions for abuse, domestic violence, mental health, and substance use issues.
• Promotes professional growth through mentoring and committee involvement.
• Supports department and organizational goals.
