Job summary

Requires a high school diploma, 3 years of pharmacy tech experience or 2 years with national certification; Tennessee licensure and PTCB med history cert within 1 year. Full-time, day shift.

Qualifications

• Three years' experience as a Pharmacy Tech or two years with national certification.
• Certification by Pharmacy Technician Certification Board (PTCB) preferred.
• State PTCB Med History Certificate must be obtained within one year.
• Previous outpatient pharmacy experience including prescription transcription encouraged.

Responsibilities

• Performs medication history assessments for new admissions with complete details.
• Obtains allergy history and documents reactions clearly.
• Determines compliance of medication consumption prior to admission.
• Enters patient’s preferred outpatient pharmacy into the electronic record.
• Navigates the electronic health record to update medication histories.
• Maintains accuracy with attention to detail and follows up as needed.
• Communicates effectively with nurses, pharmacists, and prescribers.
• Reports errors about medication history to supervising pharmacist.
