Job summary

Montefiore Medical Center in Bronx, NY is seeking a Per Diem Registered Pharmacist to join our hospital pharmacy team. The role involves dispensing medications, ensuring accuracy, and supporting clinical pharmacotherapy.

The ideal candidate will hold a NYS Pharmacist license, have at least three years of hospital experience or a pharmacy residency, and participate in continuing education. Per diem schedule: day shift, 8:30 AM–4:30 PM, with an $80 hourly rate.

Qualifications

Graduation from an accredited School of Pharmacy (BS or Doctor of Pharmacy).
Active NYS Pharmacist license and registration is required.
Minimum of three years’ experience as a pharmacist in a hospital setting or completion of Pharmacy residency.
Maintains professional competence through continuing education (15 CE credits/year).

Responsibilities

Preforms all duties of a Pharmacist position without additional support.
Dispenses medications using accepted standards for unit dose distribution and sterile/non-sterile compounding.
Reviews medication orders for accuracy and therapeutic appropriateness.
Assists neonates, pediatrics, adolescents and geriatrics with age-appropriate pharmacotherapy.
Monitors drug therapy effectiveness and documents results.
Participates in rounds and discusses drug-related issues with the care team.
Provides drug information to physicians, patients, and families.
Inspects medication storage areas per procedures and audits monthly.
Trains and orients pharmacy interns, residents and staff.
Directs other pharmacy personnel as assigned.
Participates in performance improvement and pharmacovigilance activities.
Delivers admission histories and discharge counseling as needed.
