Job summary

Montefiore Health System in New York seeks a licensed pharmacist to join our acute care team. You will dispense medications, ensure accuracy, and participate in patient care rounds with physicians, nurses and social workers to optimize pharmacotherapy across diverse patient populations.

The ideal candidate holds NYS Pharmacist license, has at least three years of related experience, and stays current with CE requirements.

Qualifications

• Graduate from an accredited School of Pharmacy with a BS in Pharmacy; PharmD preferred.
• Maintain NYS Pharmacist license and continuing education credits.
• Experience with clinical information systems and pharmacy automation.

Responsibilities

• Performs full scope of pharmacist duties without additional support.
• Prepares and dispenses meds for unit dose, ambulatory, and sterile/non‑sterile compounding.
• Reviews orders for accuracy and therapeutic appropriateness.
• Interprets data across neonatal to geriatric patients and adjusts therapy as needed.
• Participates in multidisciplinary rounds to discuss drug‑related issues.
• Provides drug information to physicians, patients and families.
• Performs monthly inspections of medication storage areas.
• Trains pharmacy staff, interns and residents.
• Engages in performance improvement and departmental committees.
