Job summary

Qualifications

• Bachelor's degree in Pharmacy or Pharm.D. from ACPE accredited school.
• Active Wisconsin Pharmacist License or eligibility with 6-month licensure window.
• Experience: none required; 1 year hospital or 3 years other pharmacy preferred.

Responsibilities

• Promote safe and effective medication use and optimize therapy.
• Collaborate with the health care team and address medication therapy issues.
• Ensure compliance with Joint Commission/MCW standards and quality improvement plans.
