Qualifications

• Pharmacist licensure in good standing and eligible for state licensure.
• Minimum of 2 years of recent hospital pharmacy experience.
• EPIC experience required.
• BLS certification if required by state regulations.

Responsibilities

• Prepare, dispense, and verify medications accurately and safely in a hospital setting.
• Review medication orders, monitor therapy, and identify drug interactions, contraindications, and allergies.
• Support medication reconciliation by gathering home medication histories and assisting with discharge medication lists.
• Provide patient and family counseling on prescribed medications at discharge.
• Contribute to antimicrobial stewardship through culture review and medication recommendations.
• Monitor and manage pharmacokinetic and anticoagulation dosing when appropriate.
• Maintain compliance with pharmacy regulations, safety standards, and documentation requirements.
• Participate in quality assurance, drug utilization review, and pharmacy committee activities.
• Assist with training, communication, and ongoing education for staff as needed.
• Support a safe, efficient, and patient-focused pharmacy service.

Job Requirements

• Current pharmacist licensure and completion of all educational requirements for state licensure
• Minimum of 2 years of recent hospital pharmacy experience
• EPIC experience required
• BLS certification if required by state regulations
• Strong clinical knowledge in antimicrobial stewardship, pharmacokinetic dosing, and anticoagulation monitoring
• Experience with medication reconciliation and discharge counseling preferred
• Excellent communication, organization, and critical thinking skills
• Ability to work independently while supporting a collaborative acute care team
