Job summary

Jackson PharmacyPros is seeking a licensed pharmacist for a contract position in Kentucky. You will apply clinical expertise to ensure safe distribution of medications for patients with diverse physical and mental health needs.

Qualifications

• Must have a valid state license to apply.
• Experience with medication reconciliation for discharge medication lists.
• Ability to gather prior-to-admission home medication lists from patients, families, or pharmacies.
• Ability to communicate discharge counseling with patients and families.
• Knowledge of antimicrobial stewardship and interpretation of culture results for recommendations.
• Knowledge of pharmacokinetic dosing management and monitoring.
• Knowledge of anticoagulation dosing management and monitoring.

Job description

We're hiring a licensed pharmacist for a contract position where you'll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges.

Minimum Requirements:

• Must have State License to Apply
• EPIC experience
• Experience with medication reconciliation for discharge medication lists
• Communication skills for gathering prior-to-admission home medication lists from patients, families, or pharmacies
• Communication skills for discharge counseling with patients and families
• Clinical knowledge of antimicrobial stewardship, including interpretation of culture results for recommendations
• Clinical knowledge of pharmacokinetic dosing management and monitoring
• Clinical knowledge of anticoagulation dosing management and monitoring
