Job summary

Virginia Commonwealth University Health Systems is seeking a Patient Flow Coordinator to optimize admission, bed management and patient transfers across affiliated hospitals and community partners through the One Call Center.

The role requires RN licensure in Virginia, a nursing diploma or associate degree, and experience in acute care; you will coordinate care pathways, support EMTALA compliance, and work 36 hours weekly across shifts including some weekends.

Qualifications

  • Diploma or Associate degree from nursing program; must be eligible for Virginia RN licensure.
  • All Patient Flow Coordinators without a BSN must enroll in RN to BSN within 2 years and complete in 5 years.
  • Master’s degree in Nursing or related field preferred.

Responsibilities

  • Coordinate patient flow across hospitals to ensure timely bed placement and transfers.
  • Serve as clinical resource for complex patient care and bed assignments.
  • Collaborate with physicians, nurse managers, clinical coordinators, charge nurses, EVS and transport teams.
  • Coordinate acceptance and bed placement for transfer referrals with Bed Management.
  • Coordinate outbound transfer referrals with Lifecom.
  • Support psych intake referrals with psychiatric admissions representatives.
  • Manage communications through One Call Center and ensure continuity of care.
  • Assist physicians in determining appropriate level of care based on admission criteria.
