The Hospital Liaison LPN facilitates referrals, coordinates skilled and non-skilled services, and other community resources through collection and input of data, interview and team collaboration in a variety of settings. The Hospital Liaison Nurse LPN performs tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered professional nurse. • High School Diploma or GED required. AAS
• Associate of Applied Science LPN preferred.NYS Drivers License upon hire required.
