Eddy Visiting Nurse Association (VNA) is seeking a dedicated Licensed Practical Nurse (LPN) to join our Intake Office team. This role coordinates patient referrals and guides them from receipt to scheduling and admission, in collaboration with an Intake Secretary and the interdisciplinary team.

You will verify orders, ensure complete documentation, and maintain accurate records in electronic systems while upholding patient confidentiality and high-quality service.

Responsibilities

Eddy Visiting Nurse Association (VNA) is seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) to join our Intake Office team. This position plays a vital role in coordinating patient referrals and ensuring a smooth transition from referral receipt to scheduling and admission. The LPN will work collaboratively with an Intake Secretary and interdisciplinary team members to process referrals, communicate with healthcare providers, and ensure all required documentation is complete to support timely patient care.

Responsibilities

• Receive and process patient referrals for home care services.
• Communicate with referral sources, physician offices, hospitals, and other healthcare providers.
• Obtain and verify physician orders and required referral documentation.
• Review referral information for completeness and accuracy.
• Prepare referral packets for admitting clinicians and scheduling staff.
• Collaborate with the Intake Office team to facilitate efficient patient admissions.

Qualifications

• Required - Current and active New York State Licensed Practical Nurse (LPN) License.

• Ability to work effectively in a fast-paced team environment.

Preferred

• One (1) to two (2) years of acute care, home care, or nursing home experience.
• Experience coordinating patient care, referrals, or admissions processes.

Hourly Compensation

Hourly Compensation: $24.00 - $31.00 hourly. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
