Job Description

Under the supervision of a Registered Nurse, coordinate the Plan of Care for patients in the home, hospital, or other settings. Work cooperatively with other team members to maintain standards of professional nursing practice, meet age‑specific needs, and operate within the scope of practice for Licensed Practical Nurses in New York State.

Education and Credentials

• Associate of Applied Science (AAS) degree in Licensed Practical Nursing required.
• Valid NYS License as a Practical Nurse (LPN) required upon hire.
• Valid NYS Driver's License required upon hire.

Experience

One year of experience in acute medical/surgical nursing preferred.
