Infinity Healthcare Consulting is seeking a Hospital Liaison to ensure smooth admissions from acute and post-acute settings. You will generate and manage referrals, build relationships with discharge planners, and drive initiatives that grow census and market exposure.

The role requires daily on-site engagement, timely communication, and collaboration with leadership to resolve delays and provide definitive answers to referrals.

Qualifikationen

• GED or HS diploma with 2+ years in healthcare or related field.
• Preferred Associate degree in healthcare, business or related field with 1 year experience.
• Must meet health assessment requirements regulated by the state.
• Capable of performing the essential functions of the job.

Aufgaben

• Stationed daily at primary account to maximize market engagement and referral review efforts.
• Review and update CRM Dashboard for accurate reporting via Forecast and other communication platforms.
• Respond to all incoming referrals on Careport and/or other referral platforms timely, daily, and throughout the day.
• Provide definitive answers for all incoming referrals within an hour of receiving and/or by end of day, if appropriate.
• Communicate and collaborate with Business Development and Operations leadership to expedite delays, barriers, and solutions in real time.
• Collaborate with AD Concierge on daily updates regarding all pending referrals for Forecast.
• Collaborate with Transitional Advisors on communication with incoming referrals and admissions on CarePort and other platforms.
• Timely PCC data input including patient demographics, resident representative, POA, and Guardian information.
• Weekly Reports
• Participate in Quarterly Business Development meeting to align initiatives with census growth and market exposure.
• Complete daily Forecast for Team review via Census call, Morning meetings and/or standdown.
• Overseeing Guest Relations and Concierge initiatives including patient rounding, testimonials, and promoting positive outcomes.

The Hospital Liaison has an essential role in ensuring a seamless transition for all admissions from acute or post acutecare. Your collaborative engagement is significant to coordinate admissions and communication to maintaincontinuity, and ensure the best care, our communities have come to expect. You will generateand manage referrals and admissions from hospitals. You will take lead developing relationships with hospital dischargeplanners, and developing initiatives that align with market to grow business and identify new opportunities. Hospital Liaisonswork very closely with our AD Concierge and leadership, to drive daily admissions. Time Management, andmulti- tasking will be necessary for market growth. Our staff and residents will rely on your knowledge and detail toachieve the best patient experience.
