Job summary

MemorialCare is seeking a Licensed Clinical Social Worker to join the Social Work Services team in Laguna Hills. This per-diem, day-shift role focuses on psychosocial assessment, discharge planning, and coordinating community resources within a hospital setting.

The LCSW collaborates with the interdisciplinary team to support patients and families, ensuring timely interventions, ethical practices, and culturally competent care across hospital areas.

Qualifications

• 2 years of Acute Hospital experience as an LCSW highly preferred.
• Community Licensed Social Work experience desirable.
• Master’s degree from an accredited School of Social Work required.
• Current California LCSW license required.

Responsibilities

• Provide clinical social work services to patients, families and groups.
• Conduct psychosocial assessments and crisis intervention.
• Assist with discharge planning and resource management.
• Collaborate with interdisciplinary team to educate and refer to community programs.
• Maintain confidentiality and ethical practices.
• Facilitate communication among care teams and families.
