Qualifications

• 1-2 years discharge planning experience in an acute care hospital setting or skilled nursing facility preferred.
• Bachelor’s degree in social work or related field preferred.
• Licensed Vocational Nurse license preferred.

Responsabilités

• Coordinate and execute discharge plans for a designated caseload of patients.
• Apply knowledge of community resources to meet discharge needs.
• Communicate with patients, families, providers, and payor sources.
• Maintain knowledge of regulatory requirements related to discharge planning.
• Prioritize and multitask to ensure timely transitions.

Formation

Bachelor’s degree in social work or related field

Licensed Vocational Nurse (LVN) license
