Job summary

You will coordinate services, assist with discharge planning, and advocate for patients, ensuring access to community resources and seamless transitions of care.

Qualifications

• CA Licensed Clinical Social Worker.
• Minimum 1 year of LCSW experience in an acute care setting.
• Behavioral Health Unit experience preferred.
• Understanding of hospital utilization review, care coordination, discharge planning, and community services.
• Excellent listening and verbal/written communication skills.
• Strong problem-solving and professional judgment.

Responsibilities

• Collaborates with the healthcare team to develop and implement care plans.
• Identify patient needs and coordinate linkage with community and in-home services.
• Assist with discharge planning and ongoing care arrangements.
• Monitor quality and appropriateness of patient care.
• Report and analyze patient satisfaction data.
• Promote optimal outcomes through psychosocial services within hospital policy.
• Complete documentation and maintain clinical records during the patient’s stay.
