MemorialCare in Long Beach seeks an experienced Licensed Clinical Social Worker (LCSW) to remove psychosocial barriers and support safe transitions through the continuum of care. The role includes supervision of staff and coordination with the care team to empower patients toward self-determination.

Responsibilities include conducting psychosocial assessments, coordinating resources, and educating families. CA LCSW license and MSW required; 3–5 years in acute care preferred.

Formación

• MSW degree required.
• Licensed Clinical Social Worker (LCSW) in CA required.
• 3–5 years direct social work experience in an acute healthcare setting preferred.
• Ability to provide clinical and casework supervision to MSW colleagues preferred.

Responsabilidades

• Manages a caseload and assesses psychosocial factors affecting treatment and discharge.
• Provides crisis intervention and coordinates with healthcare team.
• Educates and coordinates with patients, families, and community resources to prevent readmission.
• Supervises staff and provides weekly supervision to MSW staff and interns.
• Advocates for patient self-determination and coordinates services across care settings.
