The position requires a Pharm.D. or Bachelor of Pharmacy, Wisconsin license, 1 year hospital experience, and offers a $6,000 sign-on bonus, 40-hour standard weeks, and shifts days/evenings with weekends and holidays as needed.

Qualifications

• Bachelor's degree in Pharmacy or Pharm.D. from ACPE-accredited school is required.
• ASHP PGY1 or specialty residency is preferred.
• Active Wisconsin Pharmacist License is required or eligible to obtain within 6 months.
• 1 year hospital experience or 3 years other pharmacy experience preferred.
• Proficient computer skills with MS Word, Excel, and Outlook.

Responsibilities

• Promote safe, effective, and cost-conscious medication therapy.
• Collaborate with healthcare team to address medication issues.
• Ensure quality compliance with Joint Commission/MCW standards.
