Qualifications

• Bachelor's degree in Pharmacy from ACPE-accredited school.
• Current Registered Pharmacist license in good standing in the state of practice.
• Knowledge to evaluate medication therapy for patient populations served.
• Ability to evaluate therapy for safety, interactions, dosing, and disease state.
• Proficiency with pharmacy computer systems and MS Office tools.

Responsibilities

• Review medication orders for appropriateness based on patient condition and factors.
• Provide clinical oversight of regimens for the patient populations served.
• Process medication orders including entry, compounding, labeling, and documentation.
• Provide drug information and guidance to physicians, nurses, and staff.
• Ensure medications are available to minimize delays in therapy.
