UnitedHealth Group is seeking a Clinical Pharmacist, Patient Care to support hospital pharmacy operations in Cincinnati, OH. The role collaborates with physicians, nurses, and staff to ensure safe, effective medication therapy, with a focus on compliance, accuracy, and patient safety.

The position is Per Diem/PRN with onsite work arrangements and standard pharmacy responsibilities, including medication order review, therapy oversight, and clinical guidance.

Qualifications

• Bachelor's degree in Pharmacy or higher from ACPE-accredited school.
• Current Registered Pharmacist license in good standing in the state of practice.
• Knowledge to evaluate and oversee medication therapy for patient populations.
• Ability to evaluate medication therapy for safety, interactions, dosing, and organ function.
• Proficiency with pharmacy computer systems and general computer applications.

Responsibilities

• Review medication orders for appropriateness based on patient condition and factors.
• Provide clinical oversight of medication regimens for patient populations served.
• Process medication orders including entry, labeling, and documentation.
• Provide drug information and clinical guidance to physicians, nurses, and other professionals.
• Ensure medications are available to minimize delays and support outcomes.
• Delegate preparation activities to pharmacy technicians per laws and policies.
• Review products prepared by technicians and supervise technicians/interns.
• Maintain medication inventory and assist with formulary management.
• Maintain records to ensure accountability and compliance.
• Participate in performance improvement initiatives and accreditation activities.
