Job summary

HonorHealth Thompson Peak Medical Center is seeking a Case Management professional to coordinate care across inpatient and outpatient settings. You will work with patients, families, and care teams to ensure smooth transitions, proper discharge planning, and access to community resources.

The role emphasizes advocacy, collaboration, and adherence to standards of care. The position is day shift (8:00am - 4:30pm), Monday to Friday with rotating weekends, located at Thompson Peak Pkwy Scottsdale,

Qualifications

• Master's in Social Work is required; preferred candidates have advanced credentials.
• 1 year as a Licensed Social Worker is required.
• 2 years of case management experience preferred.
• 3 years as a Licensed Social Worker preferred.

Responsibilities

• Coordinate care across inpatient/outpatient episodes to ensure smooth transitions.
• Communicate discharge plans, physician and payer information, and community resources.
• Collaborate with healthcare teams to optimize patient outcomes and cost-effective care.
• Advocate for patients and families within applicable laws and guidelines.
