Job Description

We offer a full-time, remote position with home office flexibility and local engagement. Candidates must live in the Lakeland, FL area for daily in-person patient visits.

As a Field Case Manager, you will work closely with treating physicians/providers, employers, customers, legal representatives, and the injured/disabled person to create and implement a treatment plan that returns the injured/disabled person back to work appropriately, ensure appropriate and cost-effective healthcare services, and achieve maximum medical recovery and return to an optimal level of work and functioning. Responsibilities include:

  • Demonstrate knowledge, skills, and competency in the application of case management standards of practice.
  • Use advanced knowledge of injury types, medications, comorbidities, treatment options, and job duties to advise on treatment plans.
  • Interview disabled persons to assess recovery, including occupational or non-occupational injuries.
  • Collaborate with physicians/providers to create tailored treatment plans.
  • Work with employers and physicians to modify job duties for early return to work.
  • Evaluate and modify case goals based on patient progress.
  • Manage workload independently, prioritize cases, and decide on case management strategies.
  • Attend appointments, prepare status updates, and perform other duties as assigned.
Qualifications
  • Associates or Bachelor’s Degree in Nursing or related field.
  • 2+ years clinical practice preferred; workers’ compensation experience is a plus.
  • Ability to advocate effectively, work independently, and utilize basic computer skills (Excel, Word, Outlook).
  • Active RN license in good standing.
  • URAC-recognized case management certification (e.g., CCM, CRC, CRRN, COHN, CMC).
  • Reliable transportation and internet; must be able to travel locally.
Benefits

Our total compensation package includes Medical, Dental, Vision, HSA/FSA, Life Insurance, 401(k), Tuition Reimbursement, and more. The base salary ranges from $62,834 to $85,000 annually, plus potential bonuses based on performance. Benefits eligibility may vary by employment status.
