Southcoast Health is seeking a Peer Recovery Coach-Addiction to support hospitalized adults with substance use disorders. You will work with Addiction Medicine physicians, APs, and social workers to provide bedside support and recovery planning.

Role involves appointment preparation, proactive post-discharge outreach, and follow-up tasks after training. Compensation is based on experience and will include benefits where applicable.

Formación

• High school diploma plus relevant coursework in community health.
• Cultural competency and knowledge of the communities served.
• Two years’ experience with substance use and behavioral health patients.
• Experience working in a multidisciplinary team and independent decision making.

Responsabilidades

• Provide bedside support to hospitalized adults with substance use disorders.
• Assist with recovery planning and goal setting with patients.
• Prepare appointments and coordinate post-discharge outreach.
• Engage patients with peer support and follow-up after discharge.
• Participate in training and reassessments for ongoing care.
