Job summary

Infinity Healthcare Consulting seeks a Hospital Liaison to ensure seamless admissions from acute or post-acute care, generating referrals and building relationships with hospital discharge planners. The role aims to align market initiatives to grow business and identify new opportunities.

You will work closely with the AD Concierge and leadership to drive daily admissions and deliver exceptional patient experiences.

Qualifications

• GED or High School Diploma; 2 years of experience required.
• Preferred associate degree in healthcare, business or related field with 1 year of experience.
• Must meet health assessment requirements regulated by the state.

Responsibilities

• Stationed daily at primary account to maximize market engagement and referral review.
• Review and update CRM Dashboard for accurate reporting via Forecast and other platforms.
• Respond to all incoming referrals on Careport and other referral platforms promptly.
• Provide definitive answers for all incoming referrals within an hour or by end of day.
• Communicate/Collaborate with Business Development and Operations leadership to expedite delays and solutions in real time.
• Collaborate with AD Concierge on daily updates regarding all pending referrals for Forecast.
• Collaborate with Transitional Advisors on communication with referrals and admissions on CarePort and other platforms.
• Timely PCC data input including patient demographics, resident representative, POA, and Guardian information.
