Job summary

The Briarwood Community seeks a Hospital Liaison/Screener to join Knollwood Nursing Center in Worcester, MA. This role evaluates prospective patients at area hospitals for admission to the sub-acute unit, assesses care needs, and supports placement decisions to ensure high-quality outcomes.

You will collaborate with hospital teams, physicians, HMOs, and families while promoting Knollwood’s services and building referral relationships. This is a part-time role with weekends as needed.

Qualifications

• Current RN, LPN, LICSW, or LCSW credentials.
• At least 1 year of experience in long-term care and hospital/geriatric services.
• Experience with patient interviewing, assessment, and evaluation.
• Knowledge of care considerations for a wide range of illnesses/disease conditions.
• Understanding of pharmacy/rehabilitation implications for patient care and reimbursement.
• Strong interpersonal and communication skills with residents, families, physicians, hospital staff, HMOs.
• Proficiency with computers and standard word-processing applications, including MS Word.
• Strong organizational and time-management skills.
• Ability to plan, prioritize, and work independently.
• Ability to clearly articulate evaluation results and recommendations verbally and in writing.

Responsibilities

• Evaluate patients at area hospitals for admission to Knollwood’s sub-acute unit.
• Assess patient needs and care requirements for appropriate placement and outcomes.
• Review patients in relation to HMO contracts and reimbursement considerations.
• Consult with nursing/therapy leadership to determine placement and admission suitability.
• Complete required clinical and administrative documentation for admissions.
• Coordinate with hospital staff, physicians, families, HMOs during referrals.
• Represent Knollwood at hospital and HMO meetings on sub-acute policies.
