Job summary

Central Vermont Home Health & Hospice is seeking a Hospice Weekend Registered Nurse to deliver one-on-one end-of-life care in patients' homes across central Vermont. This role supports patients and families with compassionate, evidence-based care in the home setting.

We offer flexible weekend schedules with multiple variations, allowing you to balance professional growth with personal commitments. A Vermont RN license is required, with hospice/home health experience preferred.

Qualifications

  • Vermont RN license required.
  • Minimum two years of med-surg and acute care nursing experience.
  • One year of hospice/home health experience preferred.

Responsibilities

  • Provide compassionate end-of-life care in patients' homes.

Who We're Seeking:

Are you ready to make a meaningful difference in the lives of individuals and families facing end‑of‑life challenges? Our Hospice Care team is seeking a dedicated Registered Nurse to deliver personalized, one‑on‑one care in the comforting setting of patients' homes.

Schedule

We have multiple schedule variations available for this role.

  • Saturday & Sunday 8 hour days
  • Friday, Saturday & Sunday or Saturday, Sunday & Monday 8 hour days

Please note all schedules will include alternating weekend on call expectations

Your Responsibilities:
  • Provide compassionate end‑of‑life care, ensuring the physical and emotional well‑being of patients and their families.
  • Deliver expert pain and symptom management, offering comfort and support during challenging times.
  • Collaborate closely with our interdisciplinary team, local physicians, and community providers to optimize the quality of life for patients.

Requirements
  • Vermont Licensed RN with a passion for hospice and palliative care.
  • Minimum of two years clinical med‑surg and acute care nursing experience required. Minimum of one year of Hospice experience required. Home care experience and/or previous case management experience preferred.
  • Valid VT driver’s license and reliable vehicle with automobile insurance is essential for home visits.
