Job summary

Central Vermont Home Health & Hospice is seeking a Homecare Nurse Resident to lead case management for assigned patients. This trainee role is designed for new nursing graduates, or nurses with up to 2 years of experience, with a goal to become proficient within 6 months of hire.

You will develop, implement, and coordinate care plans, supervise LPNs/LNAs, participate in on-call coverage, and maintain accurate electronic documentation to support the care team.

Qualifications

  • Graduate of an accredited school of nursing with an associate or bachelor00s degree in nursing.
  • Candidate must have a current valid Vermont nursing licensure by date of employment.
  • Valid Vermont drivers license required.
  • Ability to successfully pass motor vehicle, criminal record, adult and child abuse, and Office of Inspector General checks.

Responsibilities

  • Develops, communicates, and coordinates a goal-oriented plan of care, including LNA care plan as appropriate, for each assigned patient in collaboration with patient, physician or allowed practitioner, and family, as part of an interdisciplinary healthcare team in accordance with State and Federal regulations, and CVHHH policies and procedures.
  • Implement plans of care, nursing interventions, patient/family teaching and other appropriate procedures as necessary to ensure high-quality nursing care of the home care patient.
  • Maintains up-to-date and accurate electronic documentation of care provided to ensure the integration of information for use by the heath care team.
  • Meets established nursing visit performance standards to include, but not limited to, OASIS accuracy, productivity, case mix, laptop transfers, etc.
  • Participates in primary on-call, weekend and holiday on-call, and other assignments based on agency needs.
  • Effectively supervise LPN and LNA staff as mandated by State and Federal agencies required.

Who We’re Seeking:

We are seeking for an individualized goal-oriented Homecare Nurse Resident. This position serves as Case Manager in cases in which this position is the lead discipline. This position is designed for the newnursing graduate, the nurse with preferably2 years' experience, or a nurse returning to the workforce. The expectation is to achieve ability to be proficient in the essential functions of this position within 6 months of being hired in the trainee position.
