Job summary

Beth Israel Lahey Health in Massachusetts is seeking an Evening Hospice RN to support patient-centered care Monday through Friday, 1:00pm-9:00pm. You will apply the nursing process to assess, plan, implement, and evaluate care for hospice patients and families.

You’ll complete initial assessments, develop patient-specific care plans, educate patients and families on medications and the dying process, and coordinate care with an interdisplinary team.

Qualifications

• Associate’s Degree or BSN from an accredited school of nursing.
• Active Massachusetts RN license required.
• American Heart Association BLS certification required.
• At least 1 year of direct care nursing experience, preferably in public health or home care.

Responsibilities

• Plan and deliver care using the nursing process: assessment, planning, intervention, implementation, and evaluation.
• Complete initial patient and family assessments to determine hospice needs.
• Develop patient-specific care plans with goals based on nursing diagnoses and palliative actions.
• Instruct patients and families on medications, side effects, dosage, and administration routes.
• Document services using problem-oriented records and coordinate with Hospice Aides and interdisciplinary teams.
• Supervise Hospice Aides, with bi-weekly or as-needed supervision.
• Participate in hospice case conferences.

Job Description

What You’ll Do

The Hospice RN Team Resource plans and delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with patients, significant others, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency.

More Specifically

• Complete and document an initial assessment of patient and family to determine hospice care needs.
• Perform a complete physical assessment of patient and history of current and previous illness(es).
• Provide information to the patient and family regarding the dying process and expectations.
• Develop a patient-specific plan of care, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and palliative nursing actions with emphasis on pain and symptom management.
• Provide instruction to patient and family regarding medication actions, side effects, dosage, route, and frequency;
• Provide documentation of services utilizing problem-oriented record;
• Instruct and supervise care given by Hospice Aides; Supervise the care provided by the Hospice Aide every two (2) weeks minimally, and as needed.
• Participate in bi-weekly hospice case conference or as required.

What You’ll Need

• Associate’s Degree or BSN (preferred) from an accredited school of nursing;
• An active valid Massachusetts RN license;
• An active American Health Association BLS certification;
• At least 1 year of direct care nursing experience, preferably the area of public health or home care nursing; EPIC experience is preferred.

Pay Range

$83,200.00 USD - $129,626.00 USD

The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
