Job summary

ANX Home Healthcare & Hospice Care in Santa Clara County is seeking a Visiting Registered Nurse (RN) to answer calls, assess patient needs, and perform visits according to the plan of care.

You will coordinate with hospice physicians and the Director of Patient Care, conduct comprehensive assessments, provide nursing care and education to patients and families, and participate in on-call duties and holiday rotations.

Qualifications

• CA RN license required.
• CPR for Healthcare Providers required.
• Minimum 1 year RN experience, hospice/home health preferred.
• Full-time PM shift in Santa Clara County.
• Must be driver with insured vehicle.
• Able to work with minimal supervision.

Responsibilities

• Answer calls, assess patient needs, and visit per plan of care.
• Coordinate patient care with hospice physicians and Director of Patient Care.
• Perform comprehensive patient and family assessment for hospice needs.
• Provide nursing care, emotional support, and education to patients and families.
• Participate in on-call duties and holiday rotations.
• Supervise LVNs and HHAs to ensure plan of care compliance.
