Job summary

Suncrest Hospice in the San Jose area seeks a Registered Nurse (RN) to provide compassionate care to a patient group in homes and facilities across Fremont, San Jose, and the Peninsula. You will work within an interdisciplinary team, performing psych-social assessments and coordinating with nurses, social workers, chaplains, volunteers, and CNA leadership.

The role requires current RN licensure, strong time management, independent work ability, and proficiency with electronic medical records.

Qualifications

• Current Registered Nurse (RN) licensure (in good standing) in the state of practice.
• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work closely with patients on a 1-1 level but get strong support from a patient care team.
• Can work independently and on your own from a day-to-day basis with activities including phone, email and written communication, meetings with healthcare professionals, and understanding hospice rules and regulations.
• Time management skills to manage visits, documentation, meetings, and urgent needs.

Responsibilities

• Provide care and services for a group of patients, visiting patients at homes or facilities.
• Serve as a member of the interdisciplinary team, offering psych-social assessments and community resource knowledge.
• Travel across the service area (Fremont, San Jose, Peninsula) to visit patients.
